Appointment FAQs
Yes. New patients can request dental appointments using our online form. If you have not been seen at any of our Dental Health Associates of Madison clinic locations we recommend that you complete a new patient form for your desired clinic and specialty (Family/General Dentistry, Orthodontics, Periodontics, or Endodontics).
If you are a current or new patient who would like to schedule an appointment at one of our Orthodontic, Endodontic, or Periodontal clinics you can submit your appointment request using the online form. If you know which specialist you would like to see, please enter their name in the Requested Dentist area.
